Payments can be made by check or by credit card. Credit card information can be accepted via phone.
How do I get just my immunization information?
Contact your Physician's office if you only need immunization history. They will be able to provide you this information.
What is the processing time for medical records?
Processing times vary based on the request. Routine requests are processed within 3-5 business days after payment is received.
How do I request records for a deceased patient?
Legal documentation must accompany the authorization release form. Various documents are acceptable if they clearly designate your authority as the "authorized personal representative" to request medical records. If you do not have such documentation, contact your local Family and Probate Court to inquire about becoming an "authorized personal representative". Once this documentation is received, your request can be processed. Please find more information regarding the process at https://www.mass.gov/service-details/find-out-when-its-necessary-to-probate-an-estate.
